GRANT TIMELINE

CRITERIA

  • Food Service Directors or Managers may apply for funding.
  • Applications must be for a school building. Applications for a district will not be reviewed.
  • School must participate in the National School Breakfast Program (SBP) and/or National School Lunch Program (NSLP) or equivalent USDA Child Nutrition Programs during the 2025 – 2026 school year.
  • Schools requesting equipment must serve at least one of the grades K-12.
  • Schools are awarded based on geographic location, performance on previous grants, strength of application, opportunity to increase meal participation or Average Daily Participation (ADP) and number of grant applications received.
  • Complete applications required for funding consideration.
  • Must be a school district in Michigan

High School Chocolate Milk Reimbursement Grant

Chocolate Milk is Nature’s Sports Drink, providing
13 essential nutrients for growth, development,
focus, and refueling.

GRANT OVERVIEW

Michigan High School-sponsored teams and clubs can apply for up to $500 in reimbursement for chocolate milk purchased for students to refuel. The High School Chocolate Milk Reimbursement Grant is intended to support your students’ growth, development, and education.

EDUCATION

Teams and clubs are required to watch the Sports Nutrition Video Series. To accompany the video series, we recommend ordering the FREE A Coach’s Guide to Nutrition book from our website. This book, along with other resources, can be found here: Milk Means More Material Online Order Form.

ELIGIBILITY AND GUIDELINES

  • Teams and clubs must be associated with a high school. 
  • The grant is intended for students in grades 9-12. 
  • Only one grant will be accepted per team.   
  • The school must be in Michigan. 
  • Students must watch the Sports Nutrition Video Series.  
  • The chocolate milk provided must be fat-free or low-fat.   
  • Reimbursement will ONLY be provided for the purchase of chocolate milk. UDIM will NOT reimburse other dairy products, paper products, or other items. 
  • Applicants must be associated with the school. The reimbursement check will be made payable to a school or school organization, not individuals. 
  • Applicants must complete the online High School Chocolate Milk Reimbursement Grant survey.
  • Applicants must submit (with reimbursement paperwork) at least one photo of participants receiving or drinking chocolate milk. 
  • Applicant must share at least one testimonial from a student, coach, or parent.  
  • Grant paperwork should be submitted within 30 days of the end of season or last purchase.  

REIMBURSEMENT PROCESS

  • Use this link to access the online submission form.
    Have the following items ready:  
    • A digital copy or photo of itemized receipt(s) and/or invoice(s) – Receipts must include a date, itemized list, quantities that have been purchased, total cost, and the name of the store or vendor 
    • At least one photo of students with chocolate milk (submit up to 3) 
    • School’s Agreement Number – Search for your school’s 9-digit Agreement Number at the state website: https://cepi.state.mi.us/eem/PublicDatasets.aspx   
  • The itemized receipts and reimbursement forms must be submitted within 30 days of the end of the season or within 30 days of the last purchase.  
  • UDIM will issue payment within 2-4 weeks of receiving completed reimbursement forms (with itemized receipts from the store or vendor). Receipts cannot be handwritten, a credit card authorization of payment, or a statement. 
  • Reimbursement checks will be made payable ONLY to an entity (such as a school, school district, booster organization, etc.), not a personal bank account.
